Why thought records?

The thought record is a core cognitive behavioural therapy (CBT) exercise, but paper worksheets get lost, forgotten or filled in minutes before the session. Diário RPD keeps the same structure in your patient's pocket, so records are written close to the moment and arrive organised.

  • Situation
  • Automatic thoughts
  • Emotions
  • Behaviour
  • Alternative thought

Built to support your clinical work

  • A boy with glasses holding a brain while he thinks

    Structured thought records

Situation, automatic thoughts, emotions, behaviour and alternatives, with cognitive distortions to identify.

  • A girl holding faces that show different emotions

    Emotions & mood between visits

Intensity and energy logs show how your patient felt, not only how they remember feeling.

  • A girl holding a weekly pill organizer

    Medication & reminders

Patients log doses and keep their reminders, and adherence appears next to their mood.

  • A boy showing a report with a bar chart

    Reports for your sessions

Patients share PDF and CSV reports of the entries they choose, ready to read before or during the appointment.

  • A boy hugging his diary to keep it safe

    Private & encrypted

A PIN encrypts the diary on the patient's device, and it works offline. You only see what the patient decides to share.

  • A girl holding photos from her diary

    Photos in your Check-In

Patients can add photos to a situation, giving you visual context for the thoughts behind it.

Questions therapists ask

Do my patients need an account or internet?

The diary works offline and entries stay on the patient's device.

How is my patient's information protected?

When a patient sets a PIN, the diary is encrypted on their device. Recovery codes, given once in a PDF, let them back in if they forget the PIN. You do not receive any data unless the patient shares it.

How do I get my patients' records?

Patients create PDF or CSV reports from the entries they choose and share them with you however you both prefer, for example before or during the session. Nothing is shared unless they do it.

What does Premium include?

Premium unlocks advanced reports and report periods longer than the last week. The diary itself is free.

Does it replace therapy or my clinical judgement?

No. It is a self-help tool that supports the work you do with your patients, not a diagnostic or treatment tool. What the entries mean is for you and the patient to explore together.

Which languages does the app support?

Diário RPD is available in English, Portuguese and French, so you can recommend it to patients in any of these languages.

Can patients add photos to a Check-In?

Yes. Patients can attach photos to the situation in a Check-In to keep visual context alongside their thoughts and feelings.
